In inserting the Caption, you need to click on the Numbering button and then check the box to include chapter number. You need to associate this with a particular numbered style. You would want to use the full stop (period) as a separator.

Dear Charles Kenyon, thank you for your efforts and a quick reply. Sir i was referring to this thread "https://www.msofficeforums.com/word/18190-caption-numbering-problems.html" i apologies for not mentioning it before.
I am fighting with this tiny problem from last two days. And now you can see i had made different caption for every section and sub section of the chapter manually(spent 4hrs) and tried to made index out of it but that too failed. Sir Please help me with my file, i am sharing it here with a third party hosting service. And also let me know too what was the problem and step by step procedure to rectify it.
