Default Insert a line with text right underneath

Dear Wordbros,

I am using Word 2010.

I mean to insert a line with text aligned right underneath the line, describing what is to be filled on the line. I do not know how to do this in Word and require your advice.

I intend to use the above to create "Signature lines", "Name lines", and "Date lines", to name a few examples.

I have attached a screenshot of a PDF file I have chanced upon showing clearly what I want to create in Word.

I do not want to use the "Signature Line" feature on Word, because I want more uses for the lines I insert.
