You don't need to apply any kind of editing restrictions just to use ActiveX controls. All you need do is exit the Design Mode. Applying the 'Read-only' or 'Filling in Forms' editing restrictions will, however, prevent users editing content where you don't want them to.

The 'Read-only' editing restrictions require you to specify everything a user can edit (e.g. every ActiveX control), which would include the editable portions of the last 3 pages.

The 'Filling in Forms' editing restrictions don't require you to specify everything a user can edit. Your ActiveX controls will remain editable. To designate a portion of the document as editable you need to separate it from the rest of the document by Section breaks before and after (except at the end of the document) the editable range. When applying the 'Filling in Forms' editing restrictions in such a document, a blue 'Select sections' link will appear below the 'Filling in Forms' box. You need to click on that and uncheck the Section #s for the editable sections.
