Default Saving a .dotx file brings up an old title used long ago how do I get rid of it?

Hi. I create .dotx fillable word locked files for our customer service staff to use (several people use these files daily). I use an some existing .docx file as a base, modify it changing text or adding or deleting things, change the title of the file, lock the form then select "save as" from the file menu and select my desktop as the location and .dotx from the pull down menu and save the file.

Then I open that file up and check to make sure it's filling in correction with the form parts and select "save as" again and when the dialogue box appears it has a name highlighted in blue which is a very old name that I gave to a very old file and not "document1" like it should or document2.

I thought this only happened on my machine, but a staff member asked me why it was happening and said it always gives her the same name which is "employee checklist" which was an old form I created way back when.

How do I get it to change back to what the default should be? I have looked into it and can't find an answer. Any help would be appreciated. Thanks!
