I've been using outlook 2010 for some time now on several different computers. For some reason last month on my laptop only the contact form fields got all screwed up and I don't know if I had clicked something or how it happened.

There is no longer a button for the full name and I have to click the details button on the top ribbon just to type in a name title.
Also the address is divided into separate fields in a weird way and the address button is greyed out and un-clickable.

How do I get the original contact form?
