Again, I need to preface this by saying that I don't have that version of Word in front of me (I'm using 2007, which is very different), but Tools, Customize should let you add, remove and change menus as well as toolbars (buttons). If necessary you can rebuild the whole Table menu if it's not there.
But I think an easier way it to delete your Normal template (which stores all your cusomizations). Then when you restart Word, it will recreate a clean Normal template based on its default settings, and all should be OK.
To do that, quit Word and find the file Normal.dot. It's probably in one of these locations:
  • Installed Templates:
    C:\Program Files\Microsoft Office\Templates\1033
  • User Templates Path (Default folder):
    C:\Documents and Settings\<Username>\Application Data\Microsoft\Templates
If not, you might have to do a search to find it, and you might have to show hidden files in your folder options.

You can either delete it or rename it and move it to another folder for safe keeping.
Then restart Word and all should be back to "Normal"!
