The Letter & A4 sizes refer to the paper size. If you format a document for Letter paper but print on A4, you might end up with a narrower right margin & wider bottom margin than you expect. the converse is also true. Word also has an option to allow A4/Letter resizing (see File|Options|Advanced|Print>Scale content ...) so the output is reformatted to accommodate the differences in these page sizes.
