Basic steps - ideas:
- Put the information you want to show up in your header in the body of the document.
- Use custom character styles to mark the parts you want in the header.
- Each procedure starts with a Section break - new page.
- Each Section is set to use a different first page header (which may be blank).
- In your continuation headers you incorporate your information from the body that has been marked with your custom styles using the StyleRef Field. (That was the reason for marking the text in the body with the character style.)
- In each section restart the page numbering at 1.
- Instead of a NumPages field in your page x of y format use a SectionPages field.
The first two items in your list - company name and title - could be Document Property Content Controls wherever used instead of using the StyleRef field. If changed in one spot, changes in all.

Note, with use of the StyleRef field, you will not need different Word headers in each section. The StyleRef fields will pick up the changes as they happen and modify your headers appropriately.
