For the size of the project you have in mind, the manual approach would take less time than implementing a macro solution for a project for which you've still not provided any meaningful coding information.

With your existing form, check Word's 'save form data as delimited text file' (File|Options|Advanced) option then save. You should then find it easier to copy & paste the data from the output file to your new form.
