You could do this for Word 2007 & later, by creating a custom quick part and allowing the users to insert however many of it are required. The custom quick part could contain whatever content controls are required for each record. It would also be possible to drive the quick part insertion via a macro linked to an inputBox that solicits the number required, but that level of complication might not be needed.
