Hi Tom
I have setup a sample document for your consideration which will give you what you need. This can be saved as a template if needed. The document must be saved as a macro enabled document for all the fields to work properly.
Basically what I have done is add fields to each box where the fields contain help text for that field (this is customisable). Once the user clicks on the field the help text is replaced by what is entered.
The Document create date is a standard MS Wored Field and is auto populated.
The Date of Change field uses thye standard MS Word LastSave field.
The document comprises two Sections - the first Section is protected to enable the field to work. Anything can be added below the table as this section is not protected.
