For some time I have been nagged by a prompt screen EVERY TIME I open an Office document (e.g., a Word or Excel document or any Office program). I get the prompt, "User Name. Please enter your full name and initials below:" and there is a Name and Initials filled in, with an OK button to push that launches the program file.

I have researched this issue at great length. I have uninstalled Office and reinstalled it, and the problem persists.

Does anyone else know of this issue and how to remove the recurring nag screen? I'm desperate!
