Default Semi-flexible templates (couple of questions)

Hello all,

Have been doing some searching around but so far have not found any thing to answer my question (likely meaning I'm searching for the wrong thing)!

In my office, we have a significant number of templates used for various letters, documents etc. At the top of the first page of every document, there's a banner type logo, and the return address. Is it possible to have all of our template documents automatically update that section if I change the top of a 'master' file? Functionality I suppose is similar to the PHP include command.

Also, have been trying to work out how to have a header on all pages except the first, although so far not much luck with this - a link to a good tutorial/walkthrough would be greatly appreciated.

Lastly; I've been experimenting with using fields in our forms rather than just plain white space - on the whole, it works very well having the document locked and having the user tab through the various fields.

However, on occassion, it's not quite as 'slick' as I'd hope - for example, we typically have 3 fields for the patient's name (first, middle, last). This works fine for those with only 2 or 3 names but some times there's a 4th name (they do need to be kept in seperate fields as there's several subsequent rel fields that auto-update) - is there a quicker way of adding in another field rather than unlocking, inserting, relocking, editing? Or am I getting too anal about having to use my mouse!

Any help with any of these questions/queries would be greatly appreciated.
