Thanks. It was exactly what I needed.

This is what I did if anyone ever wants to do it.

1. Insert > 2x4 table
2. Hightlight 2nd and 3rd row of the first column. Right right > merge cells.
3. Type in your info.
4. Change the font size of your word to your desire on the first column.
5. Right-click > border and spading > borders > settings: none
