You are using line numbering which counts the table as a single line.

Note that the spacing between numbers for some following lines, while correct numbering, is spaced differently from pre-printed pleading paper.

Are you required to line up your lines with line numbers? Are you required to have strict spacing?

Your rule lines are in the header. You could put your numbers inside a floating borderless text box there as well. That was how the Word 97 pleading wizard did it, I think.
