Post Paragraph Style for Thesis

Hello,

I am working on formatting my master's thesis to specifications by the graduate school and am having troubles formatting my third level subheadings. The first level is in caps and centered (easy), the second level is left justified and underlined (piece of cake), but the third level is supposed to be left justified, underlined, followed by a period and then the text immediately.

Example:

Third level heading. The rest of the paragraph begins here...etc..

Is there any way to set just the third level heading as a style so that it appears correctly in the Table of Contents automatically? The first and second level subheadings are all set, but I can't figure out how to get the third level ones. I've tried experimenting with character and list styles, but it is currently confounding me
