I keep a page on
how to prevent and remove Outlook duplicates. In the first section, it explains that two copies of Outlook running definitely causes duplicates. So, I think figuring out why two copies of Outlook are running is the first priority.
You might want to examine your list of Processes in Task Manger (via Ctrl-Alt-Delete) when you start Outlook to see how many copies are there. Also, try starting Outlook in safe mode to see if any misbehaving add-ins are causing the problem.
