I have attached a revision of your document.
On the Page Layout Tab you have a Page Setup group. Click the arrow in the right hand bottom corner which will bring up the Page Setup dialog box. Select the Layout tab at the top under Section, choose New Page, under Headers and Footers, check different first page.
In the header I have inserted a text box for your address instead of a table and positioned this behind text so it stays in the same place when you adjust the header.
Also in both the header and footer I have inserted a dashed line so they stay in position when entering information in the body of your letter. On the second page header the page number is as on the first page.
The date is right aligned, although you said you wanted it on the same line as your address, you didn't specify which line.
Paper size etc remains the same as your original.
