Hi gonner,

Word 2010 doesn't do conversions from PDF files - you need a 3rd-party application like Adobe Acrobat Pro. Alternatives:
1. You may be able to copy & paste the PDF's text into Word, then reformat to match the original layout and add the fields you require.
2. You may be able to copy & paste the PDF as an image into Word, then overlay the image with the fields you require.
