a) How do I get the TOC to be inserted with 'field codes' revealed as std. ie. I know how to reveal them but initially it's inserted as {TOC etc...}

b) Can you get more than just the page number to react as the hyperlink?

c) Can you get the hyperlink to be blue and underlined as std so that it looks like a hyperlink?
