There is definitely no userform attached to the document you posted. Indeed, being a docx file, there couldn't be one!

Having seen what you're trying to do, I'd suggest setting up the required 'fields' in an ordinary document using Word 2007's Content Controls. Unlike textbox fields on a userform, Rich Text Content Controls allow for carriage returns and, unlike formfields, support spell-checking. Best of all, no code is required.
