Answer located! The key is to use semicolons to override the sort order. What follows the semicolon is where Word would place the entry as if it were in that position.
So to make a word starting in "þ" sort in the index as if it were to begin with "tz", I use:
{ XE "Þórisfóstri;tz:Þóri's foster-son" }
