The answer is quite simple: someone has deleted the reference #s from the 1st column in Tables 1-2 to 1-4, so the cross-references to those numbers in Table 2-1 now have nothing to point to in the bookmarks concerned. In some cases, the very bookmarks referred to have been deleted.
