I'm afraid that you can't fix this with a field code switch.

The manual way to do it would involve adding bookmarks manually so that you can reference text and page numbers. It would be time-consuming and any (larger) edits made in the document would require that you recreate some of those bookmarks.

At least in theory, it can be done with a macro.
