Default Populating a text field with radio buttons or check marks

Hello all,

I'm hoping someone can help as I'm a beginner with word and excel. I am trying to create a word or excel document that has questions, which if marked yes/no, will generate continuous text in a field below. There are numerous questions with multiple parts.

Example:

Topic 1: Assessment Completion:

Did you complete your assessment?
A.) Yes
B.) Partially
C.) No

Was your assessment completed correctly?
D.) Yes
E.) No

[Text field generated for Topic 1]


Topic 2: Scheduling:

Did you complete your schedule?
A.) Yes
B.) Partially
C.) No

Was your schedule completed correctly?
D.) Yes
E.) No

[Text field generated for Topic 2]



****Text to generate for Topic 1***

If A.) is selected generate text stating "Great job completing your assessment in a timely manner."

If B.) is selected generate text stating "Your assessments are inconsistent, please work on completing thoroughly."

If C.) is selected generate text stating "You must complete your assessments in a timely manner"

If D.) is selected generate text stating "Your assessment was correct"

If E.) is selected generate text stating "However, your assessment was not completed correctly"


****Example Text Field for Topic 1 would display****

Based on selections A, E:

"Great job completing your assessment in a timely manner. However, your assessment was not completed correctly."


If anyone can help or create a sample document, I would be forever in debt!!! Thank you for any assistance!!!
