Just clarifying, the first items are not field codes but special mapped Content Controls. The page numbers are field codes. However, they are NOT on each page but rather in each section's header/footer areas (stories). Likewise, if your Content Controls are on each page, they are also in the header/foooter story rather than being used multiple times in the document. Headers and footers are parts of the document intended to print/display on each page.
I agree that using direct formatting instead of style-based formatting makes for a much larger and more complex files. Editing in Word online tends to cause problems as well.
