I will try to answer to your points -
Yes, I am using floating text boxes, because that is what word is automatically creating when I label the figures.
1. No, they are not in the same paragraph. Actually, they are on different pages.
2. Tracked revision - no, deleted captions - yes. How can I find them?
3. I did refresh the captions, but the numbers remained wrong.

Does that help? I don't know how to go from here or how to structure my content in a more regular manner.
