The StyleRef field in the header is set to pick up the text of Heading 1. The first instance of that style occurs on the third page.
The heading "Table of Contents" is in TOC Heading style so that it doesn't get included in the table contents. If you want that text to appear in the header you need to turn off link to previous in the section section, and change the StyleRef field in the header for the first section.
Sections 3 and 4 have been set to have the page number start at zero. If that isn't what you want you need to change the settings.
