You probably didn't get an answer because no-one understands exactly what it is that you want.

If you want to add ## to the start of every paragraph, do a find and replace
Find: ^p
Replace: ^p##

If your issue is in converting PDF to Word format, try different ways to convert the PDF. There are many different ways to convert PDFs to Word and each of these give different results, particularly around tabular information.
