It appears that you want five different overall ratings and I don't see how that would be useful. Have a think about how you would calculate a SINGLE overall rating. Looking at the form, I would say the two most important factors in an overall rating are probably the Current Error Rate and the Benchmark Range - are these part of your overall rating calculations.

I would also recommend you get rid of the performance check boxes and replace them with a dropdown against each of the factors. Check boxes allow users to choose more than one value which shouldn't be possible. Having five columns also doesn't leave much space for comments which a single column would solve. A dropdown would make the calculation of the Overall rating a lot simpler.
