To solve the issue, look first at your second point. Select the entire table and right click it and go to Table Properties > Row > Uncheck the "Repeat as header row at the top of each page"
Then select only the first row of the table and go back to the same place and turn this setting on just for that row.
If the table hasn't flowed back onto the previous page, select all the rows except the first row and press Alt-D to open the Paragraph formatting dialog. On the Line and Page Breaks Tab ensure that 'Keep with Next' is turned off.
It the table still hasn't flowed back onto the previous page, select the entire table again and right click for Table Properties > Table > ensure that 'Text wrapping' is set to None.
