Who are the 'people' who will be using this form?

You cannot force people to run macros, essential to achieve your aim, and such macros in a macro enabled document will cause all kinds of security issues.

As a general principle it is not ideal to hide parts of a document, as it is so easy to display the hidden parts. It is far better to only insert those parts of a document that are required. This creates the problem of where to store those parts, which brings us back to my first question which will help determine where to store those parts.
