Well, if you can't work it out, you'll need to tell us more about what kinds of data entry fields you're using and where the 'other fields' appear.

Without actually seeing a sample document, it can be difficult for anyone to diagnose the issue. You can attach a document to a sample post with some representative data via the paperclip symbol on the 'Go Advanced' tab at the bottom of this screen.
