That suggests the paragraph Style being used for the newly created paragraph is formatted as bold and underline. That could be because you've chosen the wrong paragraph Style as the one to follow whatever paragraph Style the previous paragraph was using, with a possible further complication that the following paragraph Style has the wrong attributes applied to it.
Without seeing your document, it's a bit hard to say which of these two issues might be causing the problem.
