I you have converted your document to another format and back or used a third-party program like EndNote, then you would need to re-enter them. Copy and paste can work with that but it is still a lot of work.

pdf, Googledocs and Open Office have all been known to break whatever it is that makes Word recognize its footnotes and endnotes.
