Default Preventing double EndNote reference

It seems like each time I start a new document, the footnote Endnote references are messed up. I copied text from an existing document, where the endnote number is correct, to a brand new one and it changed styles. I can't seem to fix the mess completely. Two questions:

1. How do I stop an extra, incorrectly formatted endnote reference from appearing each time I create a note? See attached .jpg.



2. How do I force Word to use the exact same styles from document to document. Endnote references should always be the same and never need adjustment, such as the font or numbering option (Roman, Arabic, etc.) I don't know why MS Word assumes that Roman numerals are the best choice. Part of my day job is helping grad students format citations correctly. Roman numerals are definitely not Chicago Manual of Style format and I've never seen this in APA or MLA either. Whoever chose this default either has no understanding of citation styles or is using some relatively rare one.
