The usual method is to restyle the content that shouldn't appear in the TOC so that the TOC isn't asked to bring it into that list.

I would be applying a different style to the headings used for the Continuation pages too. In the case of the headings, I would select the first continuation heading and make a new style called "Heading 1 NoTOC" based on that formatting. You can then apply that style to the other continuation paras. If your TOC is based on outline levels rather than style names, you will need to change this attribute on the new style.

For the extra stuff on p16, you need to bite the bullet and apply the right existing style or create new styles to deal with each instance.
