Without a template to work with within Word, the simplest solution is to use a combination of paragraph breaks, line breaks, tabs & spaces to position the text approximately where you want it.

There is also an ADVANCE field in Word for doing as you ask (see under Insert|Quickparts|Field) and this can be used to giver quite precise positioning - the units it works with are points (72pt = 1in).
