Thank you so much for your assistance. Happy to say all of the formatting issues have been rectified!

I didn't realize I had been using an older version of Word because my computer was updated with Office 365. Turns out the old version of Word was still installed and set as the default program. Yesterday, IT deleted the old version. I opened the document in Office 365 this morning and all of the formatting issues have disappeared!

Still not sure what that was about but I'm just happy its resolved.
