I don't think it was the Normal styling that was the problem. Your TOC field code appears correct but I would get rid of the \u switch which might include outline levels not already covered by the Heading 1, 2 or 3.
It appears that your Heading 3's were getting two TOC entries where the text preceded the heading number. This is not something I have come across before but would hypothesize that it might be possible by putting the text into a frame but without seeing the file, I can only guess. Another possibility is some odd way of doing cross-references but that is also conjecture.
If you Select All, Ctrl-Q, Ctrl-Space and then F9 to remove all local formatting and update the fields does the TOC resolve itself? Or did you have to remove all the body text to sort the TOC issue?
