You have a lot going on in that file. I think you will need to post a copy with just enough content to demonstrate your issue if we are to give you the simplest instructions for how you fix it.

The TOC is showing the heading 3 entries as two separate TOC paragraphs so it appears your TOC is being built with index entries rather than just with styles. If you reveal the TOC field code, what does it say? You should also go into your option settings to make sure you can see hidden text on the page.

I would also select your headings and press Ctrl-Q and Ctrl-Space to remove any local formatting. Does that resolve the multiple '6' paragraphs? Are those numbers hard typed or autonumbers?
