1. I created a table. (2x2)
2. I put a prompt in the first cell.
3. I put a Plain-Text Content Control in the second cell.
4. I repeated steps 2 & 3 in the next row of the table.
5. I selected the table and put it in a Repeating Section Content Control.
6. I saved the document.

The control to insert a table is on the left end of the Insert tab.
The ones for the Content Controls are on the Developer Tab.

I slightly modified the placeholder text in the Content Controls using the Design Mode (also on the Developer Tab) and then turning that mode off.
