It would have been helpful had you said in your original post that the desired end result was a fillable PDF form. In that case I wouldn't have bothered setting it up as a Word form, since you can't save fillable Word forms as fillable PDF forms. To get a fillable PDF form, you'll need to use Adobe Acrobat Professional or an equivalent product that can create them. Basically, what you can do is take your original PDF then overlay that with PDF fields.

For the Word form, you can toggle the formfield shading on/off via the shading button on the Legacy Forms toolbar, which you can access via Developer|Controls|Legacy Tools - you'll need to have the Developer tab on the ribbon activated to do this (see Word Options|Popular|Show Developer tab in Ribbon).
