Hi Cherry,

Your document will need re-working before it can be turned into a fillable form. Basically, what you need to do is to reformat your text boxes so they're behind the text, create a table in front of them with cell sizes to match the input areas indicated by the text boxes, then insert formfields into the table cells.

See attached. Forms protection is applied, for fill-in purposes, but no password has been used.
