Default add/remove entry in table of contents

Hello,


I have a Table of Contents in a Word document and I'm not sure how to add or remove entries. If I add lines or remove them manually, then when I do "Update entire table" my changes get rolled back. What is the proper way to create/delete entries in a TOC?
