Question Creating Test Request using Custom Form Mail Template; Separate Read Layout

Hi all,

I am trying to create a test request form that allows users to enter in relevant data that will help me with my job. The form has option buttons, check boxes, text boxes, and a date control.

The composer of the form can see all of those controls. The receiver cannot see any of the controls that were placed for the composer to use. I've included screen shots of the composing page with several active controls, the receiving page after sending the form to myself, and of the separate read layout while editing an already sent form.

The strange thing is that the subject text box, To, and CC text boxes are carried over but I've found that none of the other text boxes or checkboxes do that when I copy them over. Even when I copy the subject text box from the compose page to the read page, anything typed in the compose page in the subject textbox is carried over to the read page. But that doesn't happen for anything else. I've tried changing the values to something similar to the subject textbox. Is there some background code that allows the data entry to be carried over from the compose page to the read page?
