Default Word 2013 hyperlinks pointing to wrong location

Hello, all! Thanks in advance for the help.

My document has hundreds of hyperlinks that point to level 3 headings within the same doc. (I'm linking terms to their definitions.) I created the links by right clicking the term>hyperlink>Place in This Document>then selecting the appropriate heading.

Some links route to the wrong definition. When I open the hyperlink for editing, "Place in this Document" is no longer displayed, and instead, "Existing File or Web Page" is selected, and in the "Address" bar, it says "#_Incorrect_heading"

Editing the link to point back to the correct heading does not work- it reverts back to what I described above. Removing the hyperlink and re-building it from scratch nets the same result. (I have also tried removing and redoing all instances of a problematic term, and I still get the same problem.)

Even more confusing is in several instances, although the "Address" in the Edit hyperlink box is "#_Incorrect_heading" some of those hyperlinks do actually route to the correct definition! For example:

Text to display: "Example 1"
Address in hyperlink address: "#_Example_3"
Link routes to definition for: Example 1 (not Example 3)

The trouble seems to stem from me having made changes to my definition section. The problems are confined to definitions that have been moved or changed (or are adjacent to ones that have moved or changed). It's almost as if Word is remembering old headings and locations, even though all tracked changes have been accepted, or changes were made without tracked changes on.

I'd sure appreciate any insight into what's causing this. It's a bit hard to describe, so please let me know if I need to provide more (or better) info. Thank you!
