Default I just want to add some delimiters :)

I am trying to convert a phonebook into a contact sheet all my data is in the standard phonebook form.

Example:

Jones Al 123 Somewhere State 555-555-0023
Mike 243 Somewhere State 555-233-5434
Steve 243 Somewhere State 555-233-5434
Zach 243 Somewhere State 555-233-5434

Smith A 243 Somewhere State 555-233-5434
Bill 243 Somewhere State 555-233-5434
Dave 243 Somewhere State 555-233-5434
Ed 243 Somewhere State 555-233-5434
Frank 243 Somewhere State 555-233-5434

What I ideally want is

Jones, Al, 123 Somewhere State 555-555-0023
Jones, Mike, 243 Somewhere, State, 555-233-5434
Jones, Steve, 243 Somewhere, State, 555-233-5434
Jones, Zach, 243 Somewhere, State, 555-233-5434

Smith, A, 243 Somewhere, State, 555-233-5434
Smith, Bill, 243 Somewhere, State, 555-233-5434
Smith, Dave, 243 Somewhere, State, 555-233-5434
Smith, Ed, 243 Somewhere, State, 555-233-5434
Smith, Frank, 243 Somewhere, State, 555-233-5434

This is where I am at using find and replace functions

Jones Al 123 Somewhere, State, 555-555-0023
Mike 243 Somewhere, State, 555-233-5434
Steve 243 Somewhere, State, 555-233-5434
Zach 243 Somewhere, State, 555-233-5434

Smith A 243 Somewhere, State, 555-233-5434
Bill 243 Somewhere State, 555-233-5434
Dave 243 Somewhere, State, 555-233-5434
Ed 243 Somewhere, State, 555-233-5434
Frank 243 Somewhere, State, 555-233-5434

So I just need to get the last names to copy to the corresponding lines and place commas in there (after the last and first name).

There is a difference in the last name versus the first names that would allow this I believe. The Last name is font size 6 and the first name is 5.5. And both the first and last name are bold while the remaining text is not bold. Any help would be appreciated. Thanks!
