I don't see any evidence of a title in your attached document. What I do see is a first paragraph containing:
B, Sutton Implant Clinic, Birmingham, Onsite, Dental Implants, LF 5
and the filename:
B^J Sutton Implant Clinic^J Birmingham^J Onsite^J Dental Implants^J LF 5.docx
In the filename, it's apparent the ^J expressions have replaced the commas in the first paragraph. That is most likely an operating system issue rather than a Word/Office issue, since it is the operating system that is responsible for file naming, regardless of what application you do that from. That said, you might try repairing the Office installation (via Windows Control Panel > Programs > Programs & Features > Microsoft Office (version) > Change > Repair). If that doesn't resolve the issue, try running the System File Checker, for which see: https://support.microsoft.com/en-us/...m-file-checker
