Which steps are you using to set up the hyperlink?
In my experience, you need to know the name of the bookmark in order to link to it from outside. Even though there is a bookmark button on the hyperlink dialog, I have not been able to use it when referencing a document rather than a web page.

Type the name of the document and then append the bookmark preceded by the pound symbol #.
